A coating support form should gather enough information for a useful response without overwhelming the customer. The questions should reflect what a non-specialist can reasonably know. Unknown should be an acceptable answer when the alternative is a guess.
Request the product name, treated surface and relevant dates where available. Ask what the customer noticed, where it appears and when it began. Keep this description separate from a list of suspected causes. Allow a concise free-text account rather than forcing every case into one predefined diagnosis.
Ask about recent cleaning, maintenance products, repairs or other changes. Offer optional photographs with guidance on wide and close views. For example, an image of a localized mark should be linked to its position on the object. Avoid collecting unrelated personal information or requiring extensive documents before anyone can assess the enquiry.
Explain who will review the case and what may happen next. Keep technical questions distinct from routine purchasing enquiries where possible. Preserve the submitted information so the customer does not have to repeat the full story to every person involved. A useful form creates a coherent starting record and makes missing evidence visible; it should not pretend that form completion alone establishes the cause or remedy.
Ask for product identity, surface, treatment date, issue description, timing, recent products or repairs and relevant photos. Explain why the information is needed and provide unknown where the customer cannot answer. Avoid making a customer invent a technical diagnosis to submit the form.
Include a simple way to distinguish an order question from an application or maintenance concern. After submission, state the next step without promising an unsupported response time. Route the record to the correct reviewer and retain subsequent advice with it. A good form reduces repeated questioning while preserving the customer's original description and the facts needed for a useful assessment.
